ABSTRACT

Objective@#To investigate the characteristics of dominant bacteria of colonic lavage fluid in patients with diarrhea-predominant irritable bowel syndrome (IBS).@*Methods@#Forty-six patients with diarrhea-predominant IBS (observation group) and 46 patients with abdominal pain or discomfort who was excluded IBS (control group) from January 2016 to January 2019 in Department of Gastroenterology, Shenzhen Longhua District People′s Hospital were selected. During colonoscopy, 2 pieces of descending colonic mucosal tissues were taken by forceps, then 4 ml of colonic lavage fluid was collected from the mucosa tissues to extract total DNA, and 10 dominant bacteria were detected by real-time fluorescence quantitative polymerase chain reaction (including Bacteroides, Bacteriodes prevotella, Bifidobacterium, Clostridium, Enteropathogenic Escherichia coli, Dialister pneumosintes, Firmicutes, Faecalibacterium prausnitzii, Salmonella, Lactobacillus, and data were selected for logarithm). The correlation of dominant bacteria between colonic mucosal tissues and colonic lavage fluid was analyzed by Spearman correlation analysis.@*Results@#The composition and change of dominant bacteria of colonic mucosal tissues and colonic lavage fluid were basically the same in 2 groups. The numbers of Bacteroides, Bacteriodes prevotella, Clostridium in colonic mucosal tissues and colonic lavage fluid, and the numbers of Faecalibacterium prausnitzii in colonic lavage fluid of observation group were significantly lower than those in the control group (colonic mucosal tissues: 3.68 ± 0.54 vs. 4.34 ± 0.27, 4.26 ± 0.28 vs. 5.33 ± 0.42, 4.46 ± 0.68 vs. 4.99 ± 0.61; colonic lavage fluid: 3.26 ± 0.61 vs. 4.09 ± 0.51, 3.10 ± 1.42 vs. 4.86 ± 1.03, 5.24 ± 0.54 vs. 5.95 ± 0.51, 2.24 ± 1.83 vs. 3.24 ± 1.46), and there were statistical differences (P<0.01 or <0.05). There was no statistical differences in other dominant bacteria in colonic mucosal tissues and colonic lavage fluid between 2 groups (P>0.05). Spearman correlation analysis result showed that the Bifidobacterium, Firmicutes, Faecalibacterium prausnitzii, Lactobacillus colonic mucosal tissues were positively correlated with colonic lavage fluid (r = 0.427, 0.689, 0.577 and 0.369; P = 0.041, 0.013, 0.024 and 0.035), and Enteropathogenic Escherichia coli and Salmonella were negatively correlated with colonic lavage fluid (r = -0.352 and -0.649, P = 0.026 and 0.013).@*Conclusions@#The composition of dominant bacteria in colonic mucosal tissues and colonic lavage fluid is basically the same, and an imbalance of the microbiota in colonic lavage fluid is found in diarrhea-predominant IBS patients. The numbers of Bacteroides, Bacteriodes prevotella, Clostridium and Faecalibacterium prausnitzii significantly decrease.

Objective@#To investigate the level of foot care behaviors of in patients with type 2 diabetes mellitus in Jinshan District of Shanghai, analyze the influential factors, and provide information for the intervention study.@*Methods@#With convenient sampling method, a questionnaire survey was conducted with a sample of 110 inpatients with type 2 diabetes mellitus from Jinshan District of Shanghai. The investigation contents included patients′ demographic data, foot care knowledge and behaviors. The investigation tools were the questionnaire of diabetes general information, the questionnaire of foot care knowledge, and the questionnaire of foot care behaviors. Survey tools included general information questionnaire, foot care knowledge scale for diabetic patients and Nottingham Foot Care Assessment Scale in Chinese version. Independent-samples t test and oneway ANOVA were used in univariate analyses, whereas multiple stepwise linear regression was used in multivariate analyses. And Pearson correlation analysis was used to measure correlation between variables.@*Results@#The scores of foot care behaviors were statistically significant in the group comparison of sex and glycosylated hemoglobin(t=-2.142, F=3.071, P<0.05). The patients′score of foot care behaviors was (45.31±7.74) scores. 50.9 percent (54/106) patients′scores were less than 45. And the score of foot care knowledge was (16.10±4.18) scores, which was in the medium level. The foot care behaviors level was lower in the patients whose foot care knowledge was poorer (t=-5.591, P<0.01). Foot care knowledge, glycosylated hemoglobin level and gender were the main influencing factors on foot care behaviors of type 2 diabetic patients (R2=0.221, F=9.664, P=0.000). Pearson analysis demonstrated that there was a positive correlation between knowledge score, glycosylated hemoglobin level and behavior score (r=0.377, 0.207, P<0.05).@*Conclusions@#The status of foot care knowledge and behaviors of type 2 diabetic patients are not optimistic in Jinshan District of Shanghai. It′s very important to strengthen the education and management on diabetic patients′foot care knowledge and behaviors, especially in male patients. Meanwhile, the patients whose glycosylated hemoglobin are well controlled should not be ignored.

ABSTRACT

OBJECTIVE:To provide reference for improving the teaching quality of clinical medication nursing teaching. METHODS:The cluster sampling method was adopted to collect 8 classes of full-time undergraduate nursing students from 2011 and 2012 grade of our university(each 4 classes in each grade). They were randomly divide into trial group(analogical thought sit-uational teaching method) and control group (traditional teaching method). Horizontal comparison was conducted for multi-station comprehensive examination result evaluation and questionnaire survey. RESULTS:A total of 102 questionnaires were issued to full-time undergraduate nursing students of 2011 grade in our university,and 102 effective questionnaires were collected with recov-ery rate of 100%. A total of 102 questionnaires were issued to full-time undergraduate nursing students of 2012 grade,and 110 ef-fective questionnaires were collected with recovery rate of 100%. The average score and skill score of the students in the 2011 and 2012 grades test groups was significantly better than that in the control group,with statistical significance(P<0.05). The other items were significantly better than the students of control groupin the 2011 grades,with statistical significance(P<0.05),except for the item of"improving collaboration and organizational skills"(P>0.05). And all the students of the 2012 grade nursing profes-sional test group were significantly better than the control group, with statistical significance(P<0.05). CONCLUSIONS:It can fully embody the teaching idea of"taking students as the main body",improve students'awareness of clinical medication,nursing safety and innovation,cultivate students'clinical thinking and practical ability,and improve clinical comprehensive ability as inde-pendent thinking and autonomous learning to use analogical thought situational teaching in clinical medication nursing teaching be-fore nursing students entering the production practice.

ABSTRACT

Lack of sufficient technical supports for sharing and management of current curriculum resource, which leads to inefficiency in searching curriculum information, has become a common issue in current higher medical education. The key to best use of increasing digital medical teaching resource lies in coping with the problems of sharing and integration. A cloud based storage platform is built to manage the teaching resource of obstetrics, which enables the efficiently sharing and integration of obstetrics related courses. The achievements gained from the platform have demonstrated to improve the work efficacy of teachers and provide students with the opportunity of systematically learning, both of which ultimately con-tribute to the improvement of the quality of theoretical and clinic teaching on obstetrics and gynecology.

ABSTRACT

Children without company of parents in pediatric intensive care unit,often irritable crying,injured,or scratching themselves,pulling in all kinds of wire and/or catheter on the body,and even adverse events,such as dropping of bed.To ensure the safety of the children,we invent a multifunctional strait-jacket,and it is applied in clinic in order to solve the above problems.The structure of this strait-jacket is open,easy to install wire and all kinds of operation;shoulder,cuff with button loops fix bed guardrail by a rope,to prevent the roll shift;cuff with double fold gloves can avoid scratch the skin,remove the line,having the effect of palm warm;waist adding two constraints,not improving constraints on the waist but abdomen to keep warm.By clinical application,this strait-jacket improves the comfort of the constraint,can prevent the occurrence of complications due to improper constraints,such as skin abrasions,physical damage,blood circulation obstacle,alleviate the pain for the children.

ABSTRACT

OBJECTIVE:To evaluate the efficacy and safety of the application of airway secretions expurgation cluster manage-ment in oxygen-driven aerosol inhalation. METHODS:280 patients with chronic obstructive pulmonary disease in the Respiratory Second Division of our hospital from Mar. 2012 to Dec. 2013 were randomly divided into group A,B,C and D,with 70 cases in each group.Group A and B were treated with compound isopropyl bromide 2 ml+nomal saline 5 ml,10 min/time,twice a day,and 6 L/min oxygen-driven aerosol inhalation for 7 days;group C and Dreceived Ambroxol hydrochloride injection 15 ml+normal saline 5 ml,10 min/time,twice a day,and 6 L/min oxygen-driven aerosol inhalatiom for 7 days. group A and C were treatment groups, group B and D were control groups. Two treatment groups inhaled compound medications while the application of airway secretions expurgation cluster management. Two control groups inhaled compound medications with single effective cough guidance. RE-SULTS:Compared with control groups,the patients in treatment groups eliminated more phlegm,had lower (PaCO2)and higher (PaO2),also had a shorter hospitalization period,with significant difference(P<0.05). CONCLUSIONS:Airway secretions expur-gation cluster management technology is a simple,safe and effective method,especially in patients with chronic obstructive pulmo-nary disease in oxygen-driven inhalation therapy.

ABSTRACT

High temperature and humidity stress during seed growth and development of spring soybean can result in seed deterioration in South China. We isolated two genes (GmSBP and GmSBPL) encoding putative SBP proteins from soybean (Glycine max (L.) Merr.) to study their biological functions and response to abiotic stress,. The two SBP proteins are hydrophilic and incomplete membrane ones. Real-time quantitative (RT-PCR) analysis reveals that the expression of the two genes in the developing seeds of the seed deterioration resistant cultivar Xiangdou No. 3 and sensitive cultivar Ningzhen No. 1 was significantly affected by high temperature and humidity treatment. Meanwhile, the levels of sucrose and soluble sugar in the developing seeds of both cultivars were also affected under high temperature and humidity stress. During seed growth and development, the expression of the two genes as well as the levels of sucrose and soluble sugar reached the highest at 30 days after flower. GmSBP2 and GmSBPL were found to be differentially expressed in different soybean tissues. Sub-cellular localization indicated that two genes were located in cytoplasm and cell membrane. Our results indicate that GmSBP2 and GmSBPL might be involved in the response to abiotic stress, which will enrich our understanding of pre-harvest seed deterioration and resistance in soybean from one side.

ABSTRACT

Objective To explore the effect of extended care on behavior mental state of patients with Hirschsprung disease.Methods 40 cases of patients with Hirschsprung disease underwent surgery in our hospital from January 2009 to June 2010 were set as the control group.42 cases of patients with Hirschsprung disease underwent surgery in our hospital from July 2011 to June 2012 were set as the experimental group.The patients in the control group received conventional guidance after Hirschsprung disease surgery before discharge,which included dietary guidance,anus functional exercise,regular anal expansion,follow-up upon discomfort and regular re-visit.The patients in the experimental group not only received conventional guidance after Hirschsprung disease surgery before discharge,but also received systematic extended care plan.The nursing effect was compared between two groups.Results Conduct problems and hyperactivity index of patients in the experimental group were compared with those in the control group,the differences were significant.The scores of learning problems,psychosomatic disorders,impulsivity-hyperactivity and anxiety were lower,and compared with those in the control group,the differences were not significant.Conclusions Implementation of extended care for patients with Hirschsprung disease surgery can improve behavior mental state of them.

ABSTRACT

Objective To investigate the advantages of detection for EGFR gene mutations by denaturing high performance liquid chromatography (DHPLC) technology. Methods DHPLC was used to detect EGFR gene mutations at exon 19 and 21 in 49 cases of non-small cell lung cancer (NSCLC) patients,and the direct DNA sequencing was used to verify the accuracy of DHPLC detection. Results EGFR gene mutation was identified from 13 of 49 cases by DHPLC,including deletion mutation at exon 19 in 10 cases (76.92 %) and alternative mutations at exon 21 in 3 cases (23.08 %). Mutation results of DHPLC was consistent with DNA direct sequencing. The results of the direct DNA sequencing were the same as those of DHPLC. The sensitivity of mutation test by DHPLC was 100 %. Conclusion DHPLC technology can be used for large scale screening of EGFR gene mutation with rapid and accuracy.

ABSTRACT

Objective To explore the effect of two different enema methods in congenital megacolon children. Methods 70 cases of congenital megacolon children were randomly divided into the experimental group and the control group with 35 cases in each group respectively. The control group used traditional reflux-enema; The experimental group was implemented systemic and modified back reflux-enema (negative pressure drainage vessel to reflux-enema plus the padded-fixation of anal canals, cooperated with manual massage of lower abdomen along the direction of colon, if necessary, changed body position). The average enema time, the postoperative hospital stay and the effect of cleaning the intestine in the two groups were observed. Results Compared with the control group, the average preoperative enema time and hospitalization time was significantly shorter, and intestine cleaning effect was better in the experimental group. Conclusions Modified reflux-enema could increase preoperative enema time and postoperative hospitalization time, and had good effect on cleaning intestine.

ABSTRACT

Objective To explore the method and effect of computer simulative model of lying-in women in the teaching of gynecological nursing. Methods We separated 171 nursing students in grade 2005 into the control group (85 students) and the experimental group (86 students). For the experimental group the teaching method of computer simulative model of lying-in women was used. For the control group we used the traditional teaching method. The effects of two teaching methods were compared. Results The examination achievement of the experimental group was more than that of the control group and 93% nursing students of grade 2005 approbated this teaching method. Conclusions Introducing computer simulative model of lying-in women into the teaching of gynecological nursing can not only improve the effect of teaching, stimulate studying interest and cultivate unity and cooperation ability.

ABSTRACT

Objective To analyze the epidemud growth factor receptor(EGFR)mutations in NSCLC patients in Fujian province.Methods Fresh specimens of lung cancer and corresponding normal lung tissue were collected from 50 cases of NSCLC patients.After DNA extraction,nested polymerase chain reaction (nested PCR)and direct deoxyribonucleic acid(DNA)sequencing were used to analyze EGFR gene mutations in NSCLC patients.Results EGFR mutations in tumors were identified from 13 of 50(26%)patients,including 10 cages of in-frame deletion in exon 19 and 3 cases of amino acid substitution in exon 21.Conclusion The mjor type of EGFR mutation in NSCLC patients in Fujian is in-frame deletion in exon 19.

ABSTRACT

OBJECTIVE:To prepare and characterize capreomycin sulfate liposomes(CSL).METHODS:Capreomycin sulfate was entrapped into liposomes using active loading that pH gradient methods,ammonium sulfate gradient methods and sodium acetate gradient methods respectively followed by lyophilization technique.The liposome was characterized by entrapment efficiency,particle size,? potential and the stability.RESULTS:The entrapment efficiency of CSL pre-and post-lyophilization prepared by three methods were 65.7% and 65.2 %,20.1% and 18.6%,34.6% and 32.4%,with particle size of 136 and 145 nm,144 and 153 nm,142 and 159 nm,? potential of —20.2 and —19.5 mV,—24.4 and —22.9 mV,—18.7 and —17.8 mV respectively.No obvious changes were found in all the indexes in the stability test.CONCLUSIONS:The pH gradient technique is suitable for preparing CSL in 3 kinds of methods.

ABSTRACT

AIM: To observe and compare the antihyperlipidemia efficacy of fluvastatin vs pravasatin. METHODS: Fluvastatin group(n=41,M 22, F 19; age 60 a±s 8 a) was treated with fluvastatin 20-40 mg, po, qn for 4 wk. Pravastatin group(n=35, M 20, F 15; age 60 a±6 a) was treated with pravastatin 10 mg, po, qn for 4 wk. RESULTS: The total effective rate of fluvastatin for reducing TC was 92 %, it was higher than that of pravastatin 70 %(P<0.05). The total effective rates of flavastatin for reducing TG and raising HDL-C were 61 % and 61 %, were similar to that of pravastatin 73 % and 69 %(P>0.05). Adverse reactions occurred in 8 patients of fluvastatin(20 %) and in 7 patients of pravastatin(20 %), but were mild in both groups. CONCLUSION: Fluvastatin is a safe and effective antihyperlipidemic drug, and fluvastatin is more effective than pravastatin in decreasing total cholesterol.

ABSTRACT

Objective To evaluate the efficacy of combination treatment of montelukast sodium (singulair TM ) plus mycobacterium phlei F.U.36 (wtilin"s") on young children with asthma.Methods 96 young children with asthma were randomly assigned into GroupⅠ:montelukast sodium (orally 2 5mg,q.n) plus mycobacterium phlei F.U.36 intramuscular injection for a total of 6 ampules,Group Ⅱ only montelukast sodium orally 2 5mg nightly and placebo Group.Participants were followed up for 12 weeks.Resuts Combined treatment of montelukast sodium and mycobacterium phlei F.U.36 was more effective to improve asthma in young children than the treatment with only monstelukast sodium.Conclusion The combination therapy of monkelukast and mycobacterium phlei F.U.36 is a highly effective method for preventing asthma attacks or controlling asthma.
